Registration with the Florida Board of Medicine is required before beginning your residency program. This process requires two to three months to complete. Complete the Application for Registration as an Unlicensed Allopathic Physician. Appropriate information to be used on the application:
- Employment date: June 24, 2024
- University/hospital: Mayo Clinic
- Program specialty: Include your categorical program name
- Director of Medical Education: Margaret Johnson, M.D.
- Program address: 4500 San Pablo Road, Jacksonville, Florida 32224
- Clinical sites: Vickers Landing, Sulzbacher, St. Vincent’s, and Mission House

Submit the completed application, supporting documentation and payment directly to the Florida Board of Medicine. Mayo Clinic will provide reimbursement once you begin your training program. Your education program coordinator will assist you with the reimbursement process. Retain your receipt for submission.
