- Trainees eligible for a full M.D. license (as outlined in their appointment letter) should access the Arizona Medical Board and follow the application process. Start this process as soon as possible since it takes three months to complete. Mayo Clinic will reimburse your licensure expenses once you start your program, so keep all receipts and give them to your program coordinator. Mayo Clinic will reimburse the following licensing expenses:
- License fee
- Fingerprint fee
- Transcript fee
- Notary fee
Mayo Clinic will not reimburse fees associated with the Federation Credentials Verification Service (FCVS).
- Trainees eligible for an Arizona Medical Board postgraduate training permit (as outlined in their appointment letter), need to complete the Arizona M.D. postgraduate permit registration form, citizenship statement, and include supporting identification documents requested in the instructions sent to them via onboarding through MedHub. Mayo pays for the M.D. training permit on the trainee's behalf.
Trainees are eligible to apply for full licensure if they meet the following requirements and must do so by your appointment start date per your appointment conditions. If you do not meet the requirements (listed below) for a full Arizona Medical License, you should only complete the postgraduate training permit information in MedHub.
U.S. Graduates
- 12 months of Accreditation Council for Graduate Medical Education (ACGME) approved postgraduate training
- Completed United States Medical Licensing Examination (USMLE) Step 1, 2, 3 (M.D./DO applicants) or Comprehensive Osteopathic Medical Licensing Examination (COMLEX) (DO applicants)
Canadian Graduates
- 12 months of ACGME or Royal College of Physicians and Surgeons of Canada (RCPSC) approved postgraduate training
- Completed Medical Council of Canada Qualifying Examination (MCCQE) Part 1, MCCQE Part 2, and Licentiate Medical Council of Canada Exam (LMCC) or USMLE Step 1, 2, 3
Foreign Graduates
- 36 months of ACGME approved postgraduate training
- Completed USMLE Step 1, 2, 3

- Trainees are required to apply for a federal DEA once they are fully licensed. MCSGME reimburses all residents and fellows (except Pathology trainees) for their DEA expense fee while in a Mayo training program.
- Trainees with a full Arizona medical license that apply for their initial Federal DEA registration before starting their training program at Mayo Clinic, can use a personal credit card to pay for the DEA fee and request reimbursement with a receipt provided to their Program Coordinator when they arrive at Mayo Clinic's campus in Phoenix, Arizona.
- Trainees who currently have a Federal DEA registration in another state or other medical institution must transfer their DEA registration to the business address for Mayo Clinic Hospital in Phoenix, Arizona: 5777 East Mayo Blvd, Phoenix, AZ 85054. Note: You'll need to have a full Arizona medical license before you can change the DEA address.
- Make DEA registration transfers as soon as possible after obtaining your full Arizona medical license since this is a requirement for prescribing medications within the state of Arizona.
- If you're using this DEA at your current institution, do not transfer to Mayo Clinic until one to two weeks prior to your MCSGME appointment.
- If you currently have an institutional DEA or a fee-exempt DEA, apply for a new DEA. These types of DEAs are not transferable.
- Trainees not eligible to apply for a Federal DEA because they do not have a full Arizona medical license, but will have a postgraduate training permit, will be provided with an institutional DEA number. Your institutional DEA number will be provided at Mayo Clinic orientation.
